Back 2 School Health Clinic - North

The Back-2-School Health Clinics are returning this summer to help families across Hillsborough County prepare for the new school year. The clinics provide no-cost school physicals, routine immunizations, and Florida Form DH 680 immunization record transfers—services that are essential for school enrollment. A non-refundable $10 registration fee is required per family (not per child). Medical services are available by appointment only for children who are uninsured, underinsured, or enrolled in Medicaid.
